America is getting older and sicker. Population growth of older adults (65 and older) is unprecedented. By 2060, nearly 25% of Americans will be 65 and older, and the number of Americans 85 and over will triple. 80% of adults 65+ have a chronic condition and 68% have two or more chronic conditions. Demand for healthcare services will continue to outpace the supply of healthcare resources. How can we proactively provide more care for older Americans with fewer resources? This infographic demonstrates how RPM can play a role in treating chronic conditions.
Remote Patient Monitoring Can Transform Chronic Care
Remote patient monitoring (RPM) has the potential to fundamentally change how we provide care for chronic care patients. RPM is a proactive, preventative approach that allows your staff to care for more patients simultaneously while improving quality of care.
RPM provides continuous, remote monitoring to patients with easy-to-use, at-home, connected devices. By monitoring blood pressure, blood sugar, weight, or blood saturation (pulse ox), monitoring teams can provide timely interventions and regular coaching to improve clinical outcomes.
With unique insights into your patient’s health outside of a traditional visit, your care team members can improve the patient-provider relationship. This ultimately increases patient engagement and helps prevent ER visits or hospitalizations.
And, your practice can be reimbursed for the time your team invests in remote care by Medicare and often, Medicaid and commercial payers.
How Does RPM Work?
If you are a physician or qualified healthcare provider, you can order RPM for your patients without a diagnosis. Your team, in partnership with your RPM provider, collects patient consent and provides patient education and set-up with a device. Patients use their devices to take daily readings such as blood pressure or weight, that are automatically transferred to your office. Your clinical team monitors the data, intervening and coaching patients as needed. The RPM software should automatically track time and compliance with CPT® billing codes, so it’s easy for your team to submit for reimbursement.
If your team does not have the resources for RPM, RPM solutions, such as Optimize Health, can act as an extension of your team and provide the onboarding and monitoring services for you.
